In year 2000, Creative Rehab began in New Philadelphia, Ohio once it was realized there was a
need for better vocational rehabilitation services. Since our existence, our company has grown in size,
services offered, and positive outcomes. Creative Rehab has also expanded to include a total now of
6 Counties in Ohio including Holmes, Medina, Portage, Stark, Summit, and Tuscarawas counties.
Creative Rehab prides itself on offering flexibility and the willingness to tailor our services to each persons' unique and individual needs.

Mark Bergert
Mark Bergert is the President of Creative Rehab and is also the Divisional Director for Medina county. In addition to managing the day-to-day activities in Medina county, Mark oversees all daily operations of the business. He has a variety of work experience before entering the field of Vocational Rehabilitation in 1994. Mark has been with
Creative Rehab since 2003 and has owned Creative Rehab since 2016. Mark has a bachelor’s degree from Bowling Green State University.
Erica Jones
Erica Jones is a Divisional Director with Creative Rehab. Her territory includes: Tuscarawas and Holmes counties. She has been with Creative Rehab since 2001. She is dedicated to offering a true work experience to all individuals.
Kelly Witt
Kelly Witt is the Divisional Director for Stark county and has been with the company since 2006. Originally hired in as a Job Coach, she has taken on various roles within the company and continues to work with clients. Kelly graduated with honors from Kent State University with a bachelor’s degree in psychology.
Nikki Farnsworth
Nikki Farnsworth is the Billing Specialist for the company and has been with Creative Rehab since 2009. She appreciates the opportunity to be part of a company that focuses on servicing the community and enhancing the lives of others.
Jeff Swindell
Jeff Swindell is a Job Developer and Job Coach for Creative Rehab in Summit, Medina, and Stark counties. Jeff has a bachelor’s degree from Kent State University. After 20+ years in the banking and finance industry, he left to pursue his passion for coaching and development. Jeff has been a part of the Creative Rehab team since May of 2021. Jeff also serves as a substitute teacher in the Tallmadge City School District.
Kimberly Davis
Kimberly Davis is a Job Developer and Job Coach that has been with Creative Rehab since May of 2023. She graduated from The University of Akron with her associate degree in social work and has volunteered at different agencies within the Summit County community. Kim is returning to the community service field after 10 years of working as an event planner.
Jeanie Salmonski
Jeanie Salmonski is a Job Developer in Medina County. She joined Creative Rehab in 2023 and has worked as an Adult Disabilities Instructor, Retail Manager and Small business owner. She looks forward to empowering people through employment.
AJ Harris
AJ Harris is the Divisional Director for Portage and Summit Counties and also serves as a Job Developer. He has been with Creative Rehab since 2018. AJ has a bachelor's degree from Miami University and is pursuing his master's in education at Walsh University. Before working with Creative Rehab, AJ taught for Akron Public Schools as a long-term substitute teacher.
Daniela Sankovic
Daniela Sankovic is a Job Developer in Summit County and has been with us since 2018. Daniela has experience in job coaching, mental health, U.S. Armed Forces, and human resource case management / recruiting. Daniela has a bachelor’s degree in criminal justice, master’s degree in psychology, and doctorate degree in business administration and criminal justice. Daniela can also translate for Serbian, Croatian, Bosnian, Macedonian and Slovenian languages.
Lori Peters-McCracken
Lori Peters-McCracken is a Job Developer and Job Coach in Medina County and has been with Creative Rehab since 2018. Lori has worked in higher education for over 25 years in areas which include student advising, career services, and workforce development. She teaches in the Social Sciences and Human Services Department at Lorain County Community College. Lori earned her bachelor's degree in marketing management and her master's degree in counseling and higher education from Youngstown State University.
Gary Breen
Gary Breen is a Job Developer and has been with Creative Rehab since 2013. Gary retired from teaching after 37 years with the Plain Local Schools. Gary has a bachelor's degree from Malone University and a master's degree from The University of Akron.
Amy Lionetti
Amy Lionetti is a Job Developer for Creative Rehab in Stark and Summit counties. She has over 20 years of experience in the field of developmental disabilities, and is also a licensed substance abuse counselor. Amy joined Creative Rehab in 2023 and her knowledge of various disabilities mixed with her prior work experience will allow her to excel as a Job Developer.
Job Coaches
Creative Rehab also employs 20+ coaches to cover 6 counties. Our diverse coaching staff are highly versatile, and often have an education and/or background of working with people with disabilities. They take pride in serving our clients and our community.
